Once completed, click on the “submit” button and the Automobile Loss Notice will be electronically submitted to BHG. You will then have the option to print the completed Automobile Loss Notice for your records. Please note that submission of the Automobile Loss Notice does not guarantee coverage. Coverage will be determined after the loss information has been reviewed.
